Client Care Coordinator

San Jose, CA San Jose, CA Full-time Full-time $31 - $33 an hour $31 - $33 an hour Client Care Coordinators provide support for new and returning clients, engaging with people at different stages of cancer treatment and the family members that are also impacted by the cancer diagnosis.
Client Care Coordinators play an integral role in ensuring that no one faces cancer alone.
Duties:
Administer client intakes with new clients over the phone or in person.
Facilitate orientation calls/meetings with new clients Provide community resource information and direct referrals to other community organizations during initial call or as follow-up, finding new resources as necessary Offer ongoing referrals and resources as needed to address emerging needs Manage open cases by following up with clients in a timely manner Support clients during a Wig Boutique appointment, helping them select the right wig and head coverings to address their cancer-related hair loss Organize and maintain electronic client files Maintain working knowledge of Cancer CAREpoint programs and services Greet clients and practitioners for classes, providing hosting support for practitioners during a variety of educational and wellness programs Serve as a representative of the organization at community events by promoting Cancer CAREpoint programs, services, and community care classes Competencies:
To perform the job successfully, an individual should have experience working in a client facing role and should demonstrate the following competencies:
Bilingual in English and Vietnamese At least one year of case management or client care experience, with experience serving the needs of clients facing medical or other serious life experiences.
Experience with CRM/EHR software (examples:
Salesforce or myAVATAR).
Experience working with clients in distress Outreach and/or experience with community resource events Formal training/experience in office procedures and use of office equipment, strong computer skills.
Ability to communicate accurately and clearly both orally and in writing Broad knowledge of community organizations and resources Superior organizational, problem solving and people skills Ability to work well independently and in a group setting AA Degree from an accredited institution, or equivalent experience, required (Bachelor's degree preferred) Job Type:
